What does creatine do?
Creatine participates in the phosphocreatine system, helping buffer rapid energy demands in muscle. Its appeal is therefore tangible: can a person perform more useful training and retain more capacity? That is a narrower, more testable claim than “cellular rejuvenation”. The older-adult evidence is largely about supplementation alongside resistance exercise. [1]
What the human synthesis found
A meta-analysis of 22 studies involving 721 participants found an additional 1.37 kg of lean tissue with creatine during resistance training, with a 95% confidence interval of 0.97–1.76 kg. Strength effects also favoured supplementation. The underlying studies differed in exposure and duration, so the pooled estimate is not a promise for an individual. [1]
Why lean tissue needs interpretation
Lean tissue measurements include water and other non-fat components; they are not a direct measure of newly built contractile protein. A useful assessment pairs body composition with strength, task performance and training exposure. A change on a scan that is not accompanied by better function should not be described as equivalent to preventing frailty.
Exposure: an adjunct, not a substitute
Trials in the synthesis used different protocols across 7–52 weeks of training. That variation means the paper should be consulted for a study-specific dose rather than presenting one universal schedule. The core comparison is creatine plus training versus training alone. A result under that design cannot automatically be transferred to supplementation without exercise. [1]
Animals, safety and the longevity boundary
Animal metabolism studies can help explain energy buffering, but are not the basis of the clinical claim assessed here. Nor do the human training studies show longer survival. Safety interpretation should match the enrolled population and follow-up; a short training trial cannot resolve years of use in people with complex kidney disease, multiple medications or serious frailty.
What useful monitoring would ask
Before labelling a response successful, decide what matters: repeated chair stands, a measurable strength task, training tolerance or a relevant daily activity. Those outcomes are not interchangeable. More body mass with unchanged function would be a different result from improved performance with little mass change. The purpose of measurement is to test a claim, not manufacture a favourable number.
Trial watch and the result that would matter
The cited synthesis is completed evidence, not an active trial listing. This review has not verified a recruiting creatine trial with human lifespan as its endpoint. The most valuable next studies would test sustained function and disability in at-risk older adults, report adherence and harms, and distinguish water-related mass changes from durable adaptation.
