What are NR and NMN?
Nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) are precursors in NAD metabolism. NAD participates in cellular redox reactions and is used by enzymes involved in maintenance processes. A plausible rationale exists for studying this system in ageing. But an essential molecule is not automatically a limiting factor in every person or tissue, and raising a blood measurement does not establish improved organ function. [1]
What animal findings can and cannot establish
Preclinical work has motivated human trials across metabolic and age-related conditions. The translation depends on species, disease model, tissue and exposure. It is not justified to treat NR and NMN as interchangeable or to assume every model demonstrates longer survival. A critical review of the human NR literature is useful precisely because the strength of the preclinical narrative has sometimes exceeded the consistency of human outcomes. [1]
Three human trials, three different questions
Martens and colleagues studied NR at 1,000 mg daily for six weeks in a small randomized crossover trial. Blood cellular NAD increased and short-term tolerability was reported. The study was not a lifespan trial. [2]
Yoshino and colleagues studied NMN at 250 mg daily for ten weeks in women with prediabetes. Muscle insulin sensitivity improved, a specific metabolic result in a selected population. It does not show that all adults benefit or that a higher dose is better. [3]
Dollerup and colleagues tested 2,000 mg daily NR for twelve weeks in 40 men with obesity and insulin resistance. Insulin sensitivity and whole-body glucose metabolism did not improve. This is an important counterweight to the assumption that biochemical target engagement consistently produces metabolic benefit. [4]
Exposure is part of the evidence
The amounts above describe study arms, not a suggested supplement stack. Formulation, participant characteristics and duration differ. Combining their results into a single “NAD booster” claim loses information. A trial of NR cannot be cited as proof for a branded NMN blend, an infusion or a formulation with multiple additional ingredients. The product, exposure and claimed outcome should match the evidence. Safety: the missing long horizon
Short trials can detect common tolerability issues and changes in measured laboratory values. They cannot establish decades of safety or reliably detect rare harms. Before labelling long-term use low-risk, the evidence would need broader populations, longer observation and transparent adverse-event reporting. Existing clinical evidence is more informative about short-term exposure than prevention of late-life disease. [1]
What should a useful next trial measure?
A convincing study would prespecify who is expected to benefit, verify target engagement in the relevant tissue and measure an outcome worth improving. For example, a programme investigating muscle metabolism should also establish whether function or a meaningful clinical endpoint improves. Testing many outcomes and highlighting the best one afterwards is a weaker result. A registered protocol and independent replication would make the interpretation stronger.
Trial watch and the remaining gap
This edition does not assert a verified, currently recruiting longevity trial for NR or NMN. The linked papers provide the evidence trail for the studies discussed. The decisive gap remains human clinical benefit over a useful timescale, not a shortage of ways to raise a laboratory measurement. A future paper could improve this assessment without ever proving lifespan extension; Ethernia would record those as different achievements.
